Our goal is to help you control symptoms, manage pain and find the support you need, to help you with your Quality of Life while dealing with an advanced, chronic or terminal illness. Whether it is Hospice or Palliative Supportive Care, the Home Hospice Team is here to walk with you and your family.
We have an experienced Interdisciplinary Team who works closely with your doctor. Our patients control their plan of care and you will find our highly skilled team - medical director, nurse practitioner, nurses, social workers, chaplains, bereavement coordinator, personal care aides, administration and volunteers - are focused on your comfort and quality of life.
In addition to hospice care, we offer our Pathways Palliative Supportive Care Program for those dealing with pain control and symptoms of their disease while seeking treatment. Our grief and loss program includes support groups, 1 on 1 counseling, workshops and our popular Camp Dragonfly Grief and Loss Camp for children 8-12 years old
